To apply for the program,

1. Fill out and submit the Undersea Warriors Application.
2. Download and fill out the Divers Medical Participant Questionnaire    

*** Note *** If you or your Dive Buddy answer YES to questions 3, 5 or 10 on page 1, OR to any of the questions on page 2 of the Divers Medical Participant Questionnaire, sign and date the form AND take all three pages of the form (Participant Questionnaire and the Physician’s Evaluation Form) to your physician for a medical evaluation. Participation in the diving course requires your physician’s approval. Failure to obtain physicians approval will disqualify you from the program. When your physician approves your participation, please scan and email the form to info@underseawarriors.org
